hey @dave, i'm currently working on a web app that allows you to create and generate a base theme; it might be something you're interested in.
basically it will have a bunch of options (many which you and @kipp_brady have mentioned above) that you check to include in your theme. when you're done you just simply download it and drop it on your server.
it might be something we could work on together - i have it setup on github at the moment if you're familiar with that. (i'm developing it in very basic php)

At least they didn't print it off, annotate it, scan it back in and then email it.
I once had a client ring me up complaining he couldn't open a CD I sent him. Turns out he didn't know how to the open plastic sleeve it was in. I only understood this once he gave up and said something along the lines of "screw it, I'll just tear it open".
